2/8/2024 EnchantaVaughn Approved Behavior: The home was organized and clean when QMHP arrived. Client’s appearance was well kept up, indicating she took a bath, brushed her teeth, and took the time to iron her clothes. Client greeted her counselor in a polite and respectful way. Client and mom communicated good thoughts to each other and seemed to be in a good space, ready to participate in the family session. Client interacted safely, while QMHP expressed gratitude for her behaviors at which behaviors can last up to 10 minutes in duration. Behavior: Client spoke with high confidence and self-esteem as she talked to QMHP. Client is observed displaying age appropriate manners, avoiding distractions and impulsive responses. Client shared that she had been working on her objectives so that she can take some relief off her family who supports her daily. Client demonstrated discipline and self-control in her chair while listening to instructions and paying attention to her surroundings. Client was applauded for her leadership and taking initiative during the session at which her behaviors lasted up to 5 minutes duration. Intervention: QMHP discussed and spoke with mom and client about how they liked the services so far. QMHP discussed with the family one goal they believe they need to work on individually and as a family. QMHP brainstormed with the client on how she can keep up with her improvements and still show love to others. QMHP had client write down her goals and discuss through brainstorming and creating a goals chart on how to achieve each one. Intervention: QMHP led the client through each treatment exercise. QMHP encouraged client to be more conscious of her behaviors. QMHP provided and demonstrated empathy, compassion, and support to help her become more comfortable in the session. QMHP provided strategies such as positive reframing to educate client on how to look at things from a different perspective. QMHP assisted client with practicing reframing negative thoughts with more positive thoughts in efforts to reduce impulsive reactions to situations. Response: Client demonstrated good calming skills by deep breathing and having good voice volume in the home. Client recognized when she’s not paying attention and quickly corrected herself. Client displays good social behavior, as observed and supported by mental health professional. Client is observed making good choices as she proceeded to follow all prompts. Client is reported building healthy relationships in the home, due to client and mom interacting well with one another. Response: Client was receptive and open to the interventions, as she was attentive and focused on the information and activities that were being processed without any difficulties. Client was able to identify both positive and negative statements during the reframing skills activity. Client utilized meditation and relaxation techniques. Client reported a decrease in symptoms following the relaxation techniques. Client smiled at QMHP and said thank you, communicating that she was going to continue making good choices at home.
